are not a substitution for medical treatment. For any medical problems,
I understand that it is my responsibility to see my physician and to
have had any medical treatment completed or underway.
As her student/client, I will be taught a lifestyle of healthful living
and how to effectively utilize it in my daily life. I do recognize that
methods as administered by Solomon Faber are for the sole purpose of
assisting people to learn how to make improvements to their health and
their lives.
In addition, I hereby attest and affirm that I am here as a client, on
this and any subsequent visits, solely on my behalf and not as an agent
for federal, state or local agencies on a mission of entrapment or for
any investigative purpose. I warrant that all the information submitted
for analysis and evaluation was submitted by me and it is true to the
best of my knowledge.
